    Clinical Nurse Coordinator

    Home Health Services

    The Aga Khan University Hospital offers Home Health Services as part of its Outreach Program, providing primary to secondary healthcare services to patients in their homes across Pakistan. These services are designed to cater to children, adults, and elderly patients for general health checks or specific disease consultations.

    Key Responsibilities

    • Coordinate patient care processes and ensure timely delivery of core services.
    • Manage scheduling and staffing of individual cohorts.
    • Conduct home visits, provide patient education, and review documentation.
    • Participate in multidisciplinary rounds and plan patient care accordingly.
    • Serve as a clinical resource person for staff and patients, conducting teaching sessions as needed.
    • Provide remote assistance to staff working in home settings.

    Requirements

    • Baccalaureate Degree in Nursing (BScN) from an accredited institution.
    • Minimum three years of nursing experience.
    • Valid license from the Pakistan Nursing Council.
    • Certification in Cardiopulmonary Resuscitation (CPR), Safe Medication Administration, Aseptic Techniques, I/V Cannulation.
    • In-depth knowledge of health and safety measures.
    • Excellent communication and interpersonal skills.
    • Willingness to work in shift duties.
